For School Year 2024-2025, Legislation has passed Public Act 23-208 which requires students to be the age of 5 by September 1st in order to enroll and attend Kindergarten. Families of students who previously attended a Cromwell School, transferred out and are returning should contact Laura Mangiafico at 860-632-6046. Proof of residency and latest Physical/Immunizations will be required.
Complete the online New Registration Forms at Online New Student Registration Forms. You will create a new account first, if you do not have one.
Upload or provide the following documents. If you do not have access to upload documents, please contact Laura Mangiafico at 860-632-6047. You will need to schedule a brief meeting to provide the documents to be copied.
1. Your child's Birth Certificate (Long form of student's Birth Certificate)
2. Latest Immunization/Physical Form - Connecticut State Medical Form Submit the health form as soon as possible, as the health record must be on file prior to the student's first day of school; but do not delay the registration process while waiting to complete the physical exam
3. Proof of Residence with current date no more than 60 days old (use one of the following choices)
- Current dated Mortgage Statement showing parent's name and address (less than 60 days old);
- Escrow papers or signed mortgage commitment in parent/guardian’s name;
- Real Estate tax receipt in parent/guardian's name;
- Current Dated Utility Bill (must be Electric, Gas, Water, Sewer, Garbage Collection)
- Current dated rental agreement in parent/guardian's name or student's name;
- Notarized letter from Landlord or owner acknowledging parent/guardian’s and/or student’s residence.
4. If applicable, Copy of student's latest IEP.
5. Students latest report card and/or transcript.
